Patient Information

We understand all of our patients are unique, and we take pride in providing each individual the attention and care they deserve. We truly value our patients and place emphasis on care and respect in our practice. Your treatment and satisfaction is of the utmost importance.

What Will Happen on Your First Appointment

Unless otherwise arranged, your first appointment is for a consultation. After an examination, the nature and extent of the problem is determined and treatment options are discussed.

After you have scheduled your consultation appointment, new patient forms will be sent for you to complete at home and bring with you. If you have not been able to complete these forms, please arrive a few minutes early to complete any paperwork before seeing the doctor.

If you normally take antibiotic premedications for cleanings, please premedicate for your consultation. If you need a prescription, please call the office.

Payment Policy

Payment for the consultation appointment is due in full at that appointment, even if you have insurance coverage. The fee for the consultation will be included with any insurance filing that is done.
